I understand that I have been
charged with violating the College’s “Code of Student Conduct” and will be
subjected to a disciplinary sanction. Specifically, I am alleged to have

to any of the hearing processes listed above.
Prior to executing my
decision, I was given a copy of the institutional appeal process and TBR Policy
1:06:00:05. Further, I was encouraged to become familiar with the documents and
to seek clarification if needed from college personnel.
